The following are the labeling requirements for bags containing samples sent to us for processing. These requirements are set out in the FAHCT (Foundation for the Accreditation of Hematopoietic Cell Therapy) Accreditation Manual.
The manual states that the primary collection container must bear the following information on completion of collection :
The component's unique numeric or alphanumeric identifier - use your institutions standard identifier.
The proper name of the component in a prominent position.
The recipient's name (same as donor in our case) and a unique patient identifier number such as the hospital identifier or the social security or social insurance number, if known.
Date and time of collection (and time zone if applicable).
Approximate volume of the component.
Name and volume of anticoagulant and any other additives.
The donor's name and unique donor identifier.
Identity of collection facility or donor registry.
